Rules and Limitations
This plan is only covered at Lenox Village Dentistry and is non-transferable and it cannot be used in conjunction with another dental plan or dental insurance.
This plan cannot be used for hospitalization or hospital charges of any kind.
“Healthy gum cleaning” is defined as a routine adult prophylaxis procedure.
Any additional treatment for periodontal therapy or “deep cleaning,” including scaling and root planning is subject to additional fees.
Payment for yearly plan can either be paid in full.
No refunds will be issued at any time if the participant decides not to utilize the savings plan.
